New Patient Registration
A $75 non-refundable deposit is required from new patients to schedule an appointment online. This deposit will be applied towards the cost of the first visit and is necessary to create your account.
By clicking the Register Here button, you will be redirected to our PTEverywhere electronic records site. Please create a log in and password. After registering, you can then schedule an appointment for an evaluation.
Please remember your log in information (email and password) for your account. Your account is how you will access your invoices/superbill, schedule or cancel appointments, and view your home exercise programs.
What to expect on your first visit
You will be evaluated by a licensed doctor of physical therapy that will listen to your story, perform a full physical exam to find the source of your problems, determine a personalized plan of care based on your exam, and you will be treated on the first visit and provided a home exercise program you can start immediately.
What do I need to do before my appointment?
If you have not already done so, please complete the new patient paperwork and consent forms found in your PTEverywhere account. A link should have been emailed to the email address you provide when you registered and should be completed 24 hours before your visit for your therapist to review. This will optimize your appointment time.
What should I wear?
Loose comfortable clothing that you can move around in. Pants or bottoms should be flexible in order for your therapist to properly assess your back and hips. We kindly ask that patients avoid wearing one piece outfits like coveralls, overalls, jumpsuits or rompers.
Do I need to bring anything to my appointment?
Just yourself however, some pelvic floor patients feel more comfortable with a chaperone present at their appointment. This is entirely up to you as a patient. As this is a small clinic, we are unable to provide chaperones upon request.
If you have any imaging or tests results (X-rays, MRIs, ultrasounds, cystoscopy, urodynamic studies, etc) that you would like for your therapist to have or discuss with us about, please bring paper copies of the reports with you to your first appointment.
If you get thirsty, a bottle of water to drink. There is a bottle refill station with filtered water if refills are needed.
