We are not contracted with any insurance companies and are considered out of network. Please check with your insurance company to determine your out of network benefits for physical therapy. If you have determined that you do have out of network benefits, click below for more information on how to submit for reimbursement.

PAYMENT
We accept all major credit cards, check, and cash payments. Payment is due in full at the time of service. Online booking for evaluations/first visit will require a non-refundable $75 deposit which will go towards the first visit when scheduling online.

MEDICARE​
We are not participating providers for Medicare and cannot see Medicare patients for physical therapy. Medicare patients are eligible for wellness visits and other non covered Medicare services.

We are a cash-based practice and so are out of network with all insurances. We accept payment up front and then depending on your out of network benefit, you can pursue reimbursement if you would like.
If you are out of network and plan to seek reimbursement, we recommend calling your insurance company and asking for information on your out of network benefits. You will want to know:

1. For Tricare Select, you ARE able to submit your Superbill for reimbursement. Tricare Prime and Active Duty are not eligible for reimbursement.
2. Annual visit limit
3. Any authorization that may be required
4. If your out of network deductible has been met for the year
5. Your co-insurance after the deductible has been met

Monthly payment plans available
Payment plans offer an affordable solution to receive the care you need when you need it. You can divide your payments over multiple months. Reach out to us for more details. Some restrictions apply. Payment plans cannot be combined with insurance and thus cannot be reimbursed.
